PATIENT CONNECTED HEALTHCARE: 
ENHANCING PATIENT ENGAGEMENT THROUGH
TECHNOLOGY AND INNOVATION  
   

Engaging patients in their personal health journey is key to achieving the Triple Aim goals of improving the quality of health, improving the patient experience and reducing cost.

But how does this engagement take place?  And how do we sustain the continuum across patients, communities and clinical populations?

This half-day session will address the benefits and challenges for patients, providers, payers and technology suppliers  and include a deep dive into key topics:

  •     Where do the opportunities exist?
  •      How are healthcare organizations implementing connected health solutions in the real world?
  •      How are patients actually using their personal health data to support ownership in their care process?
  •        What role will “wearables” play?
  •        How do patient-friendly connectivity, cognitive design and education impact engagement and outcomes?
  •        Along what path will the Internet of Things (IoT) lead us?

In addition, this event will explore disruptive technologies and actionable items such as best practices, lessons learned, real-world case studies and much more.

Guest Speaker, Dr. David Lee Scher, (Bio) will be presenting on the topic "How Consumerism will drive Patient Connected Health".

Guest Speaker, Evan Keller, (Bio) will be focusing on the topic: "Beyond Step Count: Patient Friendly Connectivity for Better Outcomes." 

Guest Speaker, Michael J. Smith (Bio) will be speaking on "Patient Engagement and Technology"

Guest Speakers, Les Duncan (Bio) and Kim Suda (Bio)  will be presenting on the topic: “How Highmark Home & Community Services is leveraging data to build high performing post-acute care networks”
